Our Chronic Health & Care Passport was partly inspired by the Health Passports already being used within various NHS Trusts for people with Learning Disabilities. Easy Health have a great selection of these resources to download on a wonderfully accessible website. They are highly simplified versions of our CHCP but may be easier for some people to fill in.

UK Large Print v.01
The Standard UK Edition but with larger, clearer print.
Designed to print in full colour on both sides of A4 paper across the full page, and be stapled or hole-punched.

Best for: People with mild vision loss or dyslexia, and people with fine motor function issues who require a larger writing space.

UK High Contrast Large Print v.01
The Standard UK Edition but with large, clear print and a stripped back, black and white form with better contrast.
Designed to print in monochrome on both sides of A4 paper, with clearer title fonts and larger font sizes. Best for people with vision loss and sensory hyperstimulation, or those with dyslexia who use coloured filters. Best version for photocopying/cheaper to print.
